For students under 18, we rely on the consent of the parent/guardian to collect and use student information.

Parents/Guardians: We may disclose student information to the parent/guardian of record as listed on our client data forms.
Other Individuals: We will not disclose student information to any other individuals or entities without prior written consent from the parent/guardian (for students under 18) or the student themselves (for students 18 or older).
Required Disclosures: We may be required to disclose student information by law, court order, or in response to a valid subpoena.
Data Security:
We take appropriate measures to safeguard student information from unauthorized access, use, or disclosure.
We use secure data storage and transmission methods to protect student information.
Student/ Client Rights:

You have the right to withdraw consent for the use and disclosure of your student information at any time.
Release of Information:
Requests for release of student information must be made in writing and submitted in advance.
Please allow 48-72 hours for us to process your request and prepare the necessary paperwork.
A nominal fee may be applied for certain forms, as indicated in our practice policy.
